“Attachment, Trauma and Adoption: Clinical techniques to enhance reflective functioning”

This seminar will present a dialogue between developmental research and clinical intervention which in recent years has been elucidated in adult treatment but has received less attention in child clinical work. The focus of this talk will be to present a psychodynamically-oriented treatment case of an 9 year old girl who suffered trauma as young child, with multiple moves in foster care until she was adopted at age 7. Paying close attention to specific aspects of a psychotherapeutic intervention can point to the way that developmental research can complement clinical understanding and alert us to some possible sources for promoting therapeutic action.

Adam Phillips, British psychoanalyst and author, was Principal Child Psychotherapist at Charing Cross Hospital in London. He is now a psychoanalyst in private practice and a writer. The Independent Group in British Psychoanalysis—most notably Winnicott, Khan, Milner, Rycroft, and Laing—pioneered a unique and unprecedented approach to psychoanalytic theory and practice. Adam Phillips, identifying himself as a member and heir to the Independent Group, explores the various dimensions of the Independent Group’s approaches, while engaging with the controversies generated by them in the development of psychoanalysis. This conference concentrates on how the extraordinary range of Independent Group writing might be applicable to today’s clinical practice. As psychiatric medications have come more and more into everyday use, psychotherapists need to have an understanding of the basic principles of psychopharmacology and the uses and problems associated with the more commonly prescribed medications. The goals of this workshop are twofold: to increase the comfort level of LMSW and LCSW practitioners in working collaboratively with psychiatrists regarding medication management; and to help them guide their clients through the referral, consultation and medication management process. Psychodynamic and practical issues related to the collaborative treatment process and the giving and receiving of psychiatric medications will be integral to the course. This presentation deepens a discussion of the riveting and central issues of technique that Dr. Richards explores in his paper on self-disclosure and self-revelation. When is a self-disclosure or self-revelation an appropriate intervention? What are the historical leanings toward abstinence or toward self-disclosure? How did Freud practice—where did he stand in his formal writing, and where did he venture in his own practice? Attendees are asked to think about examples from their own practice when they have been faced with technical questions about disclosure, and to be prepared if they wish to discuss such an example. All participants are asked to read Dr. Richards’ paper prior to the presentation; it will be emailed to them once an RSVP is received. NAVIGATING BOUNDARIES AND COUNTERTRANSFERENCE WHEN WORKING WITH CHILDREN, ADOLESCENTS AND THEIR PARENTS PRESENTER: SHELLY PETNOV-SHERMAN, LCSW

Treating children and adolescents can be extremely fulfilling and fun, as well as complex and difficult. Navigating the engagement of parents while maintaining confidentiality and boundaries can feel like “walking on a tightrope without a safety net.” This presentation will help facilitate and may challenge your understanding and knowledge of the dynamics involved in working with children and adolescents and their parents. Shelly Petnov-Sherman will use clinical material to illustrate these dynamics. She will also discuss her countertransference with this group of patients and how it has impacted and informed the treatment. This presentation will include how she came to this work and the trials she has endured, as well as how her ways of working have evolved over time.

Learning Objectives:
Participants will: 1) discuss how transference and countertransference can facilitate or make for impasses in working with children, adolescents and their parents/caregivers, 2) identify nuances of confidentiality and the impact of treatment with this population, and 3) discuss the importance of acknowledging, respecting and working with racial and ethnic values with children, adolescents and their families.
