Sigmund Freud could not believe what patients told him about sexual and other forms of abuse by family members. This, together with his observations about psychosexual development, led him to over-emphasize the role of fantasy in generating emotional problems. In this communication, extensive clinical material is provided to support the thesis that what patients report about past and present impingement upon them from the outside world is equally important and needs to be taken very seriously.

Martin Silverman, M.D. is Training and Supervising Analyst and Supervising Child Analyst at The Institute for Psychoanalytic Education affiliated with NYU College of Medicine and Training and Supervising Analyst at the Center for Psychoanalysis and Psychotherapy of New Jersey. He is Associate Editor and Book Review Editor of The Psychoanalytic Quarterly as well as the author of more than 70 psychoanalytic papers and over 80 psychoanalytic book reviews.
Anna Balas, M.D. is a child, adolescent and adult board-certified psychiatrist and psychoanalyst in private practice on the Upper East Side of Manhattan. She is on the faculty at New York Psychoanalytic Society & Institute and Associate Professor on the Voluntary Faculty at The Payne Whitney Division of the New York Presbyterian Hospital and the Weill Cornell School of Medicine.
