Pregnancy and the Capacity to Permit the Emergence of a State of “Primary Maternal Preoccupation”: The Opportunity to Work Through Early Issues as an Opportunity to Effect Both Mother and Infant

Shifting from being the child of ones ‘parents to becoming the parent of ones’ child is an enormous developmental leap. Before the 1950’s when Grete Bibring and her research group recognized the enormous special anxieties of pregnant women, analysands were encouraged to leave treatment and stay at home to “bond” with their babies. Winnicott’s understanding of the need for a state of “Primary Maternal Preoccupation,” combined with recent discoveries of neurobiology have helped us understand this vulnerable challenging state and its complexity. We recognize now that treatment during this stage is an important opportunity to understand and effect the future for both parent and infant. Deep separation anxieties are stirred by the powerful wish to be a good mother as it combines with early fears and fantasies that are precipitated by body changes and the emotional meanings of parenthood.
Ellen R. Hirsch, LCSW is a Training and Supervising Analyst at the Contemporary Freudian Society and a member of the Permanent Faculty who has taught Early Ego Pathology, Infant Development, Dreams and Fantasies and Theoretical Developments after Freud. She is a graduate of the Anni Bergman, Infant Toddler program and co-author of “”The Bond of Not Knowing,” published in the International Journal of Infant Observation and its Applications.” She has presented her work on several panels sponsored by the Committee on Psychoanalysis in Clinical Social Work and was co-leader of a workshop at the International Psychoanalytic Society in Berlin on “Opportunities and Dilemmas in Working with Pregnant and Post-Partum Analysands.”
Participants and the instructor will bring clinical material from their own practices to enrich the learning process. Participants will receive a packet of four readings upon registration.
Learning Objectives:

3. Discuss the concept of a “treatment space” for pregnant women as a means of monitoring their fantasies of pregnancy as they move through pregnancy.
