CHASING AFTER THE “VAGUE AND DISTANT PATIENT”: USING THE COUNTERTRANSFERENCE & ENACTMENTS TO DEEPEN THE TREATMENT

This presentation will examine the 3 ½ year treatment of a young woman who was referred for therapy after a suicide attempt stemming from increasing depression in response to the way her boyfriend was treating her. The work involved helping the patient to gain access to her interior life so that she could begin to understand her conflicting thoughts and feelings regarding separating from her poorly attuned mother and issues of low self esteem. Most importantly, the work focused on helping the patient to begin to think symbolically so that she could gain insight into what might be keeping her so invested in the sadomasochistic relationship she participated in with this boyfriend. The patient’s dynamics – as expressed in her need to keep the therapist out and/or tie the therapist up – and her vague (and defensive) communication style unfolded in enactments and a strong countertransference experience. It was important to understand “how come” the patient maintained a dominant/submissive relationship which included explicit ritualized sexual behaviors to which she consciously consented. Utilizing the countertransference and interpreting enactments enabled the therapist to provide this patient with a holding environment and a container for her projections, informing the therapist’s understanding of the patient and the patient’s understanding of herself.

MITPP, chartered by the New York State Board of Regents, offers three certificate programs: a three year program in Psychoanalytic Psychotherapy with Adults, a two year program in Child and Adolescent Psychotherapy and a program leading to New York State licensure in Psychoanalysis for those with Masters Degrees who don’t qualify for licensure in the established mental health professions. Matriculants see patients at the Metropolitan Center for Mental Health, which is licensed by The New York State Office of Mental Health. Social Workers’ clinical hours qualify toward the LCSW. MITPP’s low tuition of $1,000 per semester includes all supervision. Courses are also offered on a non-matriculated basis.
